My Mother in Law had a terrible fall at home resulting in 4 fractured vertebrae and a tremendous fear of falling again. She was hospitalized for five days at LGPH where she received excellent care. The problem was she found not take ANY pain meds that did not result in excessive nausea and vomiting. The team finally was able to get her some relief with Celebrex and a special brace. She transferred to PHRC where she received PT and OT daily even in the weekends and we were so pleased that her mobility improved significantly! Better than when she was at home. She loved the therapy department and participated daily even when she did not always want to. As she became stronger she was able to graduate to the use of a cane instead of a walker. She also was taken home by her granddaughter where a therapist met them and did a home visit to access the home for safety modifications and her ability to get around in her own environment. She could not believe they “ would do this for me”. This really helped the family in getting her to make the modifications she needed to at home to improve her likelihood of success. She has been home now for two months and is doing very well. She said there is no place like home which of course is our goals for her as long as possible. Without the excellent therapy and nursing care we would not be at this point. Our most sincere thanks for your services, care and compassion. We highly recommend this facility for your Rehab and skilled nursing needs.

Pulaski Health & Rehabilitation Center is an exceptional facility with a top-notch team of employees who aren't afraid to go above and beyond for their patients. They genuinely care for - and about - their patients in a way that sets them apart from other facilities. They are compassionate, empathetic, and respectful to the unique needs of each patient and their family. The Nursing team takes time to learn about each patient so that they can have a more personal connection with them. Not only addressing them by name but also learning about their likes, dislikes, hobbies, etc. so that they can have meaningful conversations with them during their stay. The Therapy team at Pulaski Health & Rehab is professional and committed to helping the patient achieve their individual goals. They consistently communicate the patient's progress throughout their journey in a way that is honest, clear, and concise. They make adjustments to the therapy plan as needed to ensure that the patient is getting the most out of their therapy and continues to progress toward their goal. Dining Services works diligently to provide patients with food that is healthy and delicious and always makes sure the patients have gotten enough to eat. The dining room setting helps make patients feel more at home by offering restaurant like service where they choose from a menu and have the food brought to their table. If the patient doesn't like the choices for that meal, the dietary team has no problem fixing something else to ensure the patient stays well fed and happy. The facility stays super clean; the environmental services team does a great job ensuring that each room is cleaned thoroughly and on a regular basis. The Administration is continuously looking for ways to improve the services offered by the center, and they love to get feedback from patients and their family. I can't recommend this facility enough. If you or a loved one is in need of rehabilitation services, this shoul
