Forest City Rehab and Nursing Center

Like any Nursing home they have there ups and downs. My brother was a resident there for 8 years and considered it is home. He made many friends there and had his card group and even had a nice lady friend there. My brother was not the easiest person but they handled him very well. Tony Valentino the administrator was wonderful to deal with and they had a very good nursing staff. Like any nursing home it is up to family to visit and keep and eye on your loved ones. If I saw something I didn't like I would talk to them and they corrected it. When my brother passed they went way and beyond to make his last days comfortable and between them and OFS hospice my brother went very peaceful. They made his passing so much easier on me. Again it is up to you as a family member to keep an eye out for your loved ones and to take some responsibility. David Adamik

My brother Daniel was recently sent to Saint Anthony Hospital in Rockford with heart issues and kidney failure, He was having a hard time breathing and when paramedics brought them in it was noted that he had an oxygen tank and was on oxygen but his oxygen tank was empty I noted once again that his toenails and feet were in terrible condition and was told once before he had an appointment to go to a foot doctor, I don’t think he went, several months ago he did fall and fractured his hip but still fail to go to the foot doctor for care his hair is long and terrible looking he looks like a homeless person his fingernails were terrible and long and I just feel that he’s not getting the grooming care that he needs he has money in his account so he can get a haircut I am not sure why they don’t take him out to get a haircut I do work in a group home with 16 men are men get their haircuts monthly and are well care for Danny’s overall care is pretty good he does seem to fall a lot but hoping this this improves with the latest care that he has gotten. I do believe they may be understaffed I do like his caseworkers and the adapt personnel but I think they can only do too much for his care I plan on checking in on him often and taking him to lunch and taking him for a haircut since they can’t seem to do it there. I know where I work when our men leave our home to go to their work programs they are dressed very well very well groomed and very well taken care of
