Life Care Center of Yuma

I was sent to this facility after a hospital stay to get PT. I arrived on Sunday night at 11:45 pm. The hospital gave me enough pain meds to get thru the 3 hour ride. Once I was placed in my room I asked for pain meds and was told their pharmacy doesn’t open till 4 am. I hadn’t had pain meds since 8 pm and was supposed to get them every 4 hours. They took my own bottle of ibuprofen and gave me one. I finally fell asleep and woke up at 4:30 am in severe pain. Now I was told there was no nurse to give me any pain meds and I had to wait till 7 am. I begged for something and was told by the cna she would ask for authorization to get me something, then I got a story about how the hospital was supposed to send RX with me and didn’t, then how they wouldn’t get my RX till the next day, then how they didn’t have a Drs order. On Monday evening a Dr came by and said they needed to get an RX from Chandler AZ and it would be delivered by a driver at 10 the next morning…all this time I had no pain meds and was in extreme pain. Tuesday morning comes and no meds..Tuesday afternoon I used my walker and started yelling in the hallway that I wanted my pain meds NOW. Nurse said she didn’t know if they arrived, the Dr I saw the night before now says she never said they would arrive this day. I started screaming about wanting pain meds now! All the sudden the nurse bring me some. They denied me my pain meds for two and a half days! Other times I would ring the nurse and it would take an hour or more before they showed up…many times without my pain meds and I had to demand they go back and get it. Also my husband brought me some clothes on Monday as all I had was what I came with from the hospital. They took my clothes to be laundered…and I kept asking for my clothes…two days later I had to go down the hall screaming that I want my FN clothes and I’m not leaving…they finally gave me my clothes but lost some. Also the first day I was there they forgot to bring me breakfast and dinner…till I finally asked about it. I was told they forgot because I’m new. I talked to the administrator my first morning and he said he would get things straightened out about getting my meds but nothing ever happened. My 3rd day in I saw another Dr and he said the whole not getting my meds right away was not right…they have meds to get me and he would straighten it out. The food is absolutely horrible and barely edible. Only a couple meals were worth eating. One time I got no forks to eat with. The PT I got was the therapist coming to my room and giving me some exercises in bed and a few standing and sitting exercises. I could have just gone home and googled these but I only stayed because I thought I would get some good PT. Also I had arrived with a catheter. The staff never bothered to come clean it just empty it. I finally was able to clean it myself which was difficult due to to my bad back and the pain bending over. I had been given a walker and was using that to get out of bed. I had asked about getting the cath removed but no one would do it. Finally the PT guy took me to the nurse so I could demand it be removed which she finally did. My last night there I asked to take a shower and was told no it is not my day. I was not made aware at anytime I had to shower on specific days. I’d only had one shower the week I was there. I was never given a care plan that explained anything about shower days, food options, or the PT. This place is a terrible place to have to stay. I cannot imagine spending another day let alone being placed there to live. Thankfully I was able to advocate for myself but most residents there can’t.

Unfortunately we had a horrifying experience here. My father was here for three weeks for physical therapy. After two weeks he developed a bed sore. It wasn’t until he had a doctor’s appointment off the site that we were able to notice it. No one at the facility was aware and he had it for several days. During his appointment his blood pressure was extremely low. We notified the staff of this upon his return. His very low blood pressure went on for days. There were times when my father pushed the nurse button to get assistance to use the restroom and he waited an hour and a half for someone to respond. By week three he had fallen out of bed. The staff pretty much blamed this on him and told us unfortunately, because he requested a private room he was in a section where nurses were not close by and they couldn’t respond immediately. It was not until his fall that they decided to do labs on him and I was called later that evening that his potassium levels were very high. We had him taken to the ER and he was immediately admitted. His blood tests results were alarming to hospital staff. A day later we were told he had Sepsis. I have never left a negative comment or review on any business page. However, our concerns were brought up numerous times to staff and the management and nothing was done. Left three messages for the case manager and not once did she return my call. I am sure there are some very good nurses and staff here, I just would not recommend it after our experience.
