Oak Forest Health and Rehabilitation

My worst nightmares came true. Never did I wish to have my mother in a skilled nursing facility. When she had a stroke at age 96, we had to put her in Oak Forest for a little over 2 months. The only reason I give it 2 stars is the rehab and social worker staff. They were absolutely wonderful. The administration was so-so and did have a turnover, so maybe it will improve. The nursing staff are horrible. There were a few good RN's and CNA's but most could have cared less about their choice of work . Rude and do not care about the patients. All but 3 pieces of clothing were lost of out 8 outfits that were brand new. Her robe was given to other people. Her blanket and rechargeable OralB toothbrush did not come home. Phones in A wing were never answered. Our only way to communicate was through an Echo Show. One CNA did not realize she was on camera when she entered the room. The roommate's bell was on and she came and asked very sweetly can I help you? No, she did not need anything. My mom asked her if she could help her as she was leaving the room and she said very rudely, I'm not here for you. Well little did she know she was on camera. The interim administrator immediately responded to my concerns, but this was typical of the treatment. Depends never changed for hours. No getting out of bed on the weekends, (she could not do this herself), daily cleansing was only done on occasion. Yes, there were some caring CNA's and RN's. But the bad leave an awful taste in your mouth. The food was another concern. She was on a modified diet, but she never got what she asked for, butter, gravy, coffee with milk, tea with sugar, water etc. Covid-19 was also an issue. Many staff contacted the virus and passed it on to patients. When you cannot have contact with your loved ones due to Covid restrictions, the least the staff can do is show concern and care. Do not recommend this facility to anyone considering it.
