The Avenue at Medina

I would not even give them a one star. They left my mother with a broken hip for hours and did not get to the hospital until the middle of the night and the emergency room was shocked that the nurse on duty did not know how they did not recognize it and it boils down to pure incompetency and laziness! Also, when my mom was asking for help and was in pain the nurse on duty told me that the diabetic patients were more needy and required her attention. Mean while my mom was bleeding internally. The icing on the cake was they did not even call 911 they waited for non-emergency transport.

I put my 94 year old Aunt in the Avenue. She was there for 28 days. It is a beautiful building, but nothing was beautiful about her care. I went to see her and there sitting on her dresser, was her medication. It had sat there for several hours. They only changed my aunt 2 - 3 times a day if she was lucky. The call light was a joke. It would be on for hours without anyone coming in her room. Her room was only cleaned one time while she was there and that's only because I complained. Most of the aides and nurses were rude. I definitely wouldn't recommend putting anyone's loved one in there. Looks are very deceiving.

Do not trust your loved one into the care of this facility. My husband was there for about 4 months during the COVID lockdown. The facility paid no attention to symptoms that he was exhibiting. Communication was terrible. One week after his release from the facility I took him to the ER where they found he was malnourished in addition to other things

Excellent care!! My mom loved it there. They were able to get her home to us before the weather got cold out. She is walking like she was before she went to hospital. The nurses are great they always are prompt and smiling. I would never need to worry about anything especially during corona. Some day if I ever need rehab I will definitely pick this home.

IF YOU LOVE YOUR FAMILY MEMBER DO NOT ALLOW THEM TO STEP ONE FOOT IN THIS PLACE! My 98 year old grandmother was taken by ambulance from Medina hospital to the avenue for a stay to help her gain strength in her legs so she could use her walker again after being hospitalized for 10 days for other issues. She arrived rested and bruise free late on Friday the 10th and by Monday the 13th she had a face full of bruises, a black eye, a goose egg on her forehead, a gash by her temple, bent up glasses, physically exhausted and sad. Backing it up: When the family visited Saturday morning we had a meeting with grandmas nurse. We explained to him she was an extreme fall risk, that she needed a walker at all times to move and that she was there to build up strength in her legs. He assured us she would be evaluated by PT staff and get er underway. When I arrived Sunday morning I found my grandmother slumped over in her wheelchair next to her bed looking sad and confused. I asked her what was wrong and she told me she was so tired and that she didn't like it there she liked the hospital better. I couldn't understand her reaction so i asked her nurse how her evening was. He proceeded to tell us that my grandmother (who can't walk) was found "furniture surfing" around her room from 2 to 5 am so she didn't get much sleep. I was very concerned. I said again she is a FALL RISK, she cannot be getting out of bed, she cannot walk and where were people to help her? Apparently she was ringing her buzzer for help to the bathroom for over an hour and no one came. I asked for a walker to be put in her room, he said "No, she has to EARN IT" Monday the 6th my parents got a phone call on their answering machine at 5:40am "Hi, this is the avenue, just want to tell you your mother had a real good night last night everything is fine, bye bye" What an odd time for such a call. When my mother got to the Avenue Monday morning she was not prepared for the condition she found my grandmother in. Despondent, bruised, afraid, sad, confused and needing to go back to the ICU. Again, grandma rang her buzzer for help to the bathroom and no one ever came so she tried to go herself and fell hitting her head on the counter and ended up laying on that cold floor for who knows how long. While they were there they heard a woman yelling for help across the hall for over 10 minutes, my dad went to see what was the matter and the lady pleaded for help as she said she was left alone on the toilet for over 40 minutes and needed help. My dad had to go get someone to help her. Meanwhile we asked questions as to why my grandmother fell and why no one told us on the call. Her answer: "Yeah, I told the night nurse you were going to be upset", WHAT!!!!! ????? Upset we are furious. They lied to us!!We made two calls to the administrator who could not even apologize but tried to shame the "local Hospital" for being "worse than their facility, she clearly has heard these complaints before and I know now that we are not the only ones who have gone through this at their facility. When my husband and I went to clean out her room we noticed that other residents with bruises on their faces and our hearts just sank for them. Never again!!!

My 91 year old mother just spent two weeks at the Avenue and I chose the place because I heard it was beautiful. Beautiful it was but my mom didn’t need crystal chandeliers and grand pianos, she needed care. The care she received was absolutely deplorable. She was only showered twice a week. (We were told that’s all they allow). The response time to her nurse call button was anywhere from 30 minutes to an hour and sometimes no one came at all. One day while visiting her I timed it and it took 35 minutes. The nurse told us she didn’t have time for call lights as she was too busy passing out food trays to the building. Shouldn’t the nurses be focusing on patient care & the food service team be handing out trays. If you are looking for quality care for your loved one please please don’t be fooled by the fancy facade the Avenue portrays. With the lack of care this facility demonstrates they should be written up by the state.
